Add your first client
A client is a practice we work for. You connect one by naming a PC at that practice and running one command on it: a small Windows service reads the clinic's database there and sends the tables you choose to this Engine, over its own outbound connection.
Three steps, about ten minutes: name the PC, send whoever is at the practice one command to paste, then choose what it sends.
